Terms of Agreement

In order to protect both parties, the following agreement must be signed to ensure appropriate documentation for services rendered.
Both the client and the videographer (Katy Cooper) should feel a sense of security when there is the exchange of money for services and vise versa.
Please fill out the below statements and be sure to pay deposit within a reasonable timeframe when booking.

1. I, Katy Cooper, agree to fulfill my duties as the videographer for _________. I agree to be there for the discussed amount of time (_____ hours) to film all special moments of wedding day (date:__________) as discussed.

2. I understand that I have to pay $50 deposit, that goes towards my purchase, to finalize transaction of booking. I also understand that full payment must be paid before receiving video.(Client receives video through Google drive unless they also want flash drive)

3. I, Katy Cooper, agree to use my best abilities to provide a video to your liking. If you need an edit to your video, the first initial edit is free BUT I understand there will be a fee only if edits are extensive or there are several edits needed. This can be discussed between both parties after client has watch Katy’s original video.

4. I agree to give Katy Cooper all the information needed and/or required for before the wedding day arrives. Including the $50 deposit, a wedding invitation, and a time frame of each activity happening for that day. (Example: Bride getting ready 8:30am-10am, Groom getting ready 12pm-1pm, etc...)

5. If there is any reason that I, Katy Cooper, has to cancel before event happens and the couple has already paid money, Katy agrees to give a full refund of the amount that was received back to the client. In return, If I have to cancel with Katy Cooper, within a month of the wedding, I will have to pay Katy Cooper a cancelation fine of $100. I understand this is because a contract was broken.

6. (Circle/Underline one): I give permission to Katy Cooper for video to be used on online/YouTube if she chooses to

I DO NOT want my video to be displayed online, I would rather enjoy it privately
